Basketball Coaching Background Checks
The following clearances are required for SAFE certification and are accessible via Pennsylvania’s  DHS website.  

• Pennsylvania Child Abuse History Clearances (CY113) 

• Pennsylvania Criminal Record Checks for Volunteers (SP4-164A)

• Federal Bureau of Investigations (FBI) Criminal Background Check (only if you have lived outside of PA in the last 10 years).  If you have lived in PA for the last 10 years, you can use this document in place of the fingerprint certification. 

• CDC Heads Up Concussion Training
